Registered Nurse and Vocational Case Managers

Job Summary:

Provides medical case management and coordination of services to claimant to enable their safe return to successful employment or to reach the optimum level of wellness, independence and quality of life.  Case management is defined by CCM as a collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates the options and services required to meet an individual’s health needs, using communication and available resources to promote quality, cost-effective outcome.  This position is primary dealing with workers’ compensation, as well as STD, LTD and pharmacology review files. The Case Manager will interact with adjusters, attorneys, employers, medical personnel and the injured worker; attend local onsite medical appointments to obtain the treatment plan and coordinate a safe return to work with the employer. In addition to attending appointments, the Case Manager is required to perform timely documentation and reports using a web-based computer system.

Job Requirements:

  • Minimum 2 years Clinical Experience

  • Case Management Experience Preferred

  • Advanced Industry Credentials (i.e., CCM, CDMS, CRC, CRRN, COHN, etc) within 2 years of hire*

Job Summary:

The Case Manager provides services for employees with disabilities or medical conditions to ensure that they can safely and expeditiously either return to work or stay at work. Provides ergonomic evaluations, onsite consultation and/or management of return to work programs such as ADAAA and light duty. This position provides a communication conduit between the customer and service provider.

Provides or arranges for services for staff with disabilities or medical conditions to ensure that they can safely and expeditiously either return to work or stay at work. Ongoing consultation and support to supervisors, benefits staff and departments is provided. The following are areas of focus for this position:

  • Refinement of the early intervention process between client and the employer

  • Ensures that all work performed is documented in client’s Case Management System

  • Work with the employer to create a seamless transition for return to work.

Job Requirements:

  • Master’s degree in Rehabilitation Counseling, Social Work, Industrial Psychology, Counseling Psychology, or a related field, or Bachelor of Science Degree in Nursing

  • Ten years of experience working with people with disabilities in a return to work setting

  • Knowledge of federal and state disability laws regarding access for people with disabilities, specifically ADAAA and FMLA

  • Demonstrated knowledge of community resources for people with disabilities

Primary Responsibilities:

  • Provide information, referral, and support to employees who claim STD/LTD benefits

  • Coordinate return to work services and accommodation recommendations with the employer’s Human Resources’ benefits staff, Leave Of Absence administrators, Departmental Supervisors and employee benefit vendors

  • Promote employee self-advocacy, independence and development

  • Maintain strict confidentiality in verbal and written communications

  • Keep updated on legislation and national, state, and local issues affecting access for employees with disabilities

  • Consult with medical providers on disability duration, work capacities and release to return to work

  • Provide on-going consultation and problem-solving assistance to employees and their staff in the provision of appropriate services and accommodations and advocate for reasonable accommodation

  • Conduct job site analyses as needed

  • Conduct ergonomic assessments and make recommendations for needed accommodations for potential claims prevention claims

  • Document and maintain up-to-date case records and notes of appropriate and accurate information on the computer database and in the hard file

  • Consult with H.R. Staff, Wellness Coordinators, Safety, EAP and managers regarding case management issues when appropriate

  • Present cases for consultation at weekly staff meetings with all stakeholders

Primary Responsibilities:

  • Provide on-going consultation and problem-solving assistance staff while delivering services and accommodations and advocate for reasonable accommodation. Meet regularly with staff, department supervisors and employee benefit vendors to coordinate services.

  • Manage an early intervention and return to work process for employees with disabilities or medical conditions related to claims prevention or return to work.

  • Identify appropriate transitional duty positions based on restrictions for employee(s) in coordination with supervisors, medical providers and HR staff. Monitor transitional duty placements for progress and provide support as necessary until the employee can return to work full duty.

  • Review and consultation of ADAAA requests. Utilize outside resources as needed to provide accommodation options. Facilitate the accommodation process including overseeing installation of equipment and orientation and follow up with employee. Assist in the resolution of any challenges related to accommodations.

  • Review and consultation of disability and Workers Compensation claims for return to work. This includes providing case management services with the employee, medical provider, HR staff, supervisor and other stakeholders.

  • Conduct job site analyses as needed for specific return to work claims. Ascertain the specific physical tolerances and essential job functions for particular employment positions.

  • Conduct or coordinate through outside resources, work area assessments and make recommendations for needed accommodations for potential claim prevention, return to work or ADAAA accommodation.

  • Document and maintain up-to-date case records and notes of appropriate and accurate information within the computer case management and absence management systems.
